We recommend and maintain the following schedule of routine well child exams for our patients:
- 3 to 7 Days Old
- 2 Weeks Old
- 2 Months of Age
- 4 Months of Age
- 6 Months of Age
- 9 Months of Age
- 12 Months of Age
- 15 Months of Age
- 18 Months of Age
- 24 Months of Age
- 3 Years to 18 Years Annual Physical exams
- New Patients – For Baseline Purposes
Please Note:
In the event of rescheduling an appointment, we request you to give us 24 hours prior notice.
If any scheduled appointment is missed without prior notice, a “no-show” charge of $25.00 will be applied to your account.
All Co-pays, Deductibles are due at service.
We accept all major Credit Cards*, and also accepts Cash or Checks for your Co-pays, Co-Insurance and any Patient deductibles.
For any bounced/returned checks a charge of $35.00 will be applied to your account.
*We charge Credit Card Processing fees of up to 3%, for certain types of credit cards and payments made over the phone/online
